Mandibular Distraksiyon Osteogenezinin Biyomekanik Temelleri

Özet

Distraksiyon osteogenezi tekniği, histolojik düzeyde konvansiyonel cerrahilere göre mucizevi faydalar sağlasa da uygulanması ve takibi oldukça kapsamlı bir planlama ve dikkat gerektirmektedir.  Distraksiyon osteogenezinin başarısı doğrudan biyolojik ve biyomekanik faktörlere bağlıdır. Bu faktörler genellikle 3 başlık altında incelenmektedir; ekstrensek veya fiksatörle ilişkili faktörler; intrensek veya dokuyla ilişkili faktörler; distraktörün oryantasyonuyla ilgili faktörlerdir. Bu faktörlerin en önemlisi ve uygulama ile ilgili hataya en açık alan, biyomekanik prensipler yani distraktörün oryantasyonu olmaktadır. Kraniyofasiyal distraksiyon ile rekonstrüksüyonlarda, özellikle mandibular ilerletme veya genişletmelerde, bu prensiplere riayet etmek, istenilen yöndeki kemik kazancını sağlamak ve komplikasyonlardan kaçınmak için mecburidir.

The distraction osteogenesis technique, although providing miraculous benefits at the histological level compared to conventional surgeries, requires extensive planning and attention for application and follow-up. The success of distraction osteogenesis is directly related to biological and biomechanical factors. These factors are generally examined under three headings: extrinsic or fixator-related factors; intrinsic or tissue-related factors; and factors related to the orientation of the distractor. The most important of these factors and the area most prone to error in implementation are the biomechanical principles, namely the orientation of the distractor. In craniofacial reconstructions with distraction, especially in mandibular advancements or expansions, adherence to these principles is essential to achieve the desired direction of bone gain and to avoid complications.
